This is the Klingon Vor'cha Class with an alternate front module, based on a design sketch from Rick Sternbach he made for the Vor'cha. He envisioned the head to be an interchangeable, mission specific module, that can also act as a life boat in emergency situations. Sadly, this idea was never realized and we only saw the Vor'cha on screen just with the Disruptor Module. This is the Torpedo/Phaser module, as it features a torpedo launcher at the front and phaser strips on the small wings to the side.
