This is the USS Voyager with the ablative armor deployed, that Adm. Janeway brought back from the future in the last episode of Star Trek: Voyager "Endgame". This new model is a much more detailed version by Enrico Zellmer-Volk. Please also look at his shop at Havocs-spaceshipyard, he has awesome stuff there.
