This is based on a fan designed, TOS style Medivac Shuttlecraft. It is also called an Emergency Response Vessel, Robert Jarvik Class, on the plans that can be found on the internet. It is much larger than the original Class-F Shuttle from TOS and can carry up to 8 patients in diagnostic/medical stasis beds.
