Product Description
Pair. Tested with Samsung Odyssey, but it should work with Odyssey+.
This order includes lens adapter for both eyes. Lenses are not included. Get a pair of lenses compatible with this adapter by ordering this pair of eyeglasses for $10

If you have diopter lower then -4.0 I suggest lens material with standard index 1.50 (That is CR39 material which has lower abberation value (less color separation at the edges) and therefore better quality. Otherwise get Index 1.57 (if you are in USA you most likely have eyeglasses with this index and if you don't see color separation with your eyeglasses, this will be fine also). Add anti-reflective coating, as it is free. Don't get other coatings.

According to my research, when ordering above eyeglasses, the IPD (interpupillary distance) should be set to 65 mm regardless of your personal IPD. This way the center of eyeglasses lenses would overlap the center of the fresnel on Odyssey Mixed Reality Headset lenses, and you will get the best possible quality and least eye-strain as everything will be aligned. You can adjust IPD on Odyssey anyway.

There are two versions, Medium and Large. Medium should work up to -4.25 (or plus) in index 1.50, and up to -8.00 with index 1.57. Large was tested with +5.25 (and index 1.61)

Larger diopter is supported with higher index lenses (available on Zenni at order time, with higher prices).
If you have different diopters for each eye, you need adapter that fits larger diopter. If the difference is large send me a message to confirm that it will work or I'll upload a custom model.

If you have minus diopter, there is something called NV-Add, it's what you'd add to the Sphere values to get the right prescription for near vision. You can add +0.5 or +1 to your diopter to make it more comfortable for longer session.  This would mean if your SPH diopter is -4.5, you would order eyeglasses at -4. The headset resolution is not at eye reslolution anyway and this will also blur out the SDE a bit.

A few notes
This adapter is oval and does not follow the exact free-form curvature of the Odyssey lens assembly.
This adapter has been tested with a different 3D printer than Shapeways has so it will not look like in the images and the fit might be slightly different. You will need to pop-out the existing lens cover before mounting the adapter. It should pop-out easily, don't use sharp objects or too much force. You need to snap-in tooth on one side, and then push the other side so that the other tooth clicks into place (see the video below).\
