Short story from Fred, my sea captain friend:
"I was on a twin-screw ship merely as an observer - while the captain was trying to dock. All of a sudden she would not answer and she rammed the dock. It was shortly confirmed that one of the screws was missing - must have lost the lock nut and backed off."
