This is the upper portion of the Ministry of Magic Visitor ID Badge that is pictured on page 150 of the Film Wizardry book.
* Please note that you will have to use a piece of wire (that is not provided) to connect the two pieces of the visitor's badge. Copper wire or even a paper clip work very well.
