Truncated sphere d5, hollow, labelled in Braille. The number sign is omitted, and one side of the face is flattened to allow the reader to orient the die, to tell which way is up. The number is also provided in Arabic numerals, debossed into the face.
Unfortunately, the die lands edge-up three times out of five, and you can't easily label the edge in Braille, so I guess it will have to be read by picking it up and reading the bottom, the face it landed on. Not ideal, but certainly workable.
Note: not tested for fairness (yet).
