You use a rubber band or wristband to secure this to your wrist. The distance between the 8mm wide pegs is 163.4 mm which is pretty standard for most piano keyboards. The Octave helper should be worn as shown in the simulation so that you can still individually finger keys and not hit the octave helper against your piano when it is not needed. When you need to hit octaves, just spread your thumb and pinky apart as you normally would and the pad of your palm will do most of the work. There are notches carved out in the pegs to help keep the pegs from slipping off the black keys. If you like you can glue on a little felt to the tips to dampen the contact sound between the pegs and the keys.
