I sell iphone cases that use a metal bottle opener attachment.

I have a couple that i can start shipping out and i would like to add a small surcharge to the cases that i sell.
However, i would need the costumer's shipping address and name. This is not provided to me at all specially since personal comments can't be added to the purchased order.
I also want to make the metal attachment available at a discounted price to people who have purchased my product. I was going to set up a paypal account and ask for the order # as proof they purchased my product. However, the excell download option with this information is not working. (which needs to be fixed ASAP in my opinion)
Can anyone help? i'm pretty sure people would like this option.. like the guy who designed this ring
