When I upload an object for 3d printing sometimes the preview image has the correct UV map on part the object and sometimes it looks like the image is just projected down onto it.
The first time I printed something up in color I just added that ImageTexture line and it worked fine. Now I can't even get the same object reexported to work.
Is there some other prep in Blender that needs to be done for it to work

That's odd - it should always work the same. Could you mail me the file that gives an incorrect preview to bart@shapeways.com? I'll check it out for you.
Only adding a 'ImageTexture' to the VRML will probably not work - you'll need to fully UV-map the texture. You can do all that in Blender.
